Medical Records

Information is the lifeblood of the health care delivery system. The medical record, in manual or automated form, contains the medical information that describes all aspects of your patient care at Permian Regional Medical Center.

It is a communication link among caregivers, as well as providing data for medical research, peer review, public health studies, and substantiating reimbursement.

Mission Statement

The mission of the Health Information Management Department of Permian Regional Medical Center is to provide quality transcription, complete retrievable and accurate documentation for research, and a guide for treatment.

Birth Certificates

Our department works cooperatively with the Andrews County Clerk's office to insure that accurate information is submitted on each child born at Permian Regional Medical Center. Although the birth certificate originates at PRMC, it is not official until the County Clerk's office submits it to the State of Texas.

Advance Directives

Copies of signed and dated advance directives are retained on file when furnished to the department.

Texas Cancer Registry

As mandated by the Texas Cancer Incidence Reporting Act and the Texas Department of Health, in cooperation with the Center for Disease Control (CDC), Permian Regional Medical Center is an active participant with the Texas Cancer Registry doing our part for cancer research.

Your Medical Record

Every effort is extended to protect the information contained in a medical record of all patients of Permian Regional Medical Center. The Health Information Management Department is a secured area. Confidentiality is a priority that does not waiver.

All records are retained for the period of time specified by the State of Texas, with 10 years being the minimum. Copies of your medical records can be obtained for a nominal fee.
